Health Choice Care focuses on achieving the following four principles:
Health Choice Care is jointly owned by eighteen community health centers located throughout Florida, Missouri, Hawai’i, North Carolina, and Rhode Island.
Promoting accountability for the care of Medicare FFS beneficiaries
Requiring coordinated care for all services provided under Medicare FFS
Encouraging investment in infrastructure and redesigned care processes
The Shared Savings Program is designed to improve beneficiary outcomes and increase value of care by:
Eligible providers, hospitals, and suppliers may participate in the Shared Savings Program by creating or participating in a Medicare Accountable Care Organization (ACO). An ACO is a group of physicians, hospitals, and other providers who voluntarily join to assume responsibility for their patients' quality and total cost of care. ACO providers work together to improve healthcare quality, coordinate patient care, and reduce costs. ACOs can achieve savings by reducing costs and meeting quality requirements.
About ACOs
Eligible providers, hospitals, and suppliers may participate in the Shared Savings Program by creating or participating in an Accountable Care Organization (ACO).
